You can go to any doctor- your home doctor is fine. Our home doctor charges somewhere around $80. It's probably more now- they've really gotten ritsy.

Most recently we got ours done on I 75 just south of I 40- the first exit. Go east off the exit and there is a shopping center with a walk-in clinic. I don't remember the exact price but it was about $40 each if I'm not mistaken.

A lot of Petros and/or TAs have walk in clinics for this, too.

If you have until October, you're fine.

I got my first one done on my own dime, at my family doc. Cost me about $100 in '97. Any doctor can do the physical if you provide the form, but be careful they fill it out correctly if you go to a doc who isn't used to these. I forget the details, but there's one box they want to check that they shouldn't check. Something about needing a waiver for your diabetes.
