Here's the website, DAC is now USIS.

http://www.usis.com/commercialservices/ourservices.htm

They must furnish you with one free report if you request one. All you have to do is state that you are thinking about applying for a driving position.

Drivers wishing to obtain a copy of employment information previous employers have stored on them may send a copy of their Drivers License and Social Security Card to:

USIS Transportation Services Consumer Department
P.O. Box 33181
Tulsa, OK 74153

or visit this link: http://www.usis.com/commercialservic.../ContactUs.htm

You'll need to provide your home address and telephone number along with all correspondence. Keep a copy of your request for your own files.

If you have never driven a CMV before or it has been over 10 years since you last held a driving job you will not have a DAC report. Also if you have never worked for a DAC reporting company you will not have one.

Here is a little more info:

For those who don't know about DAC & USIS, it is the employment report version of your credit report that trucking companies use to determine whether to hire you are not. Companies can add certain information on your report such as whether you are re-hirable or not. Other things include, but is not limited to accidents you might have been involved in, and vehicle abandonments.

Their telephone number is 800-381-0645 (provided by OOIDA). If you are a CDL driver, your option is #2, but their "menu selection" is "always changing". Laughing

Here is what you will hear:

Quote:
You are entitled to one free copy of your consumer report per year.

To qualify for the free report, Any of the next 3 conditions must apply:

* 1. You are currently unemployed and intend to seek employment in the next 60 days.
* 2. You are currently receiving public welfare assistance.
* 3. You believe that your file is incorrect due to fraud.

To obtain your copy, please send a written request along with a legible copy of your Drivers License and Social Security card.

Your request must indicate which condition makes you eligible for the free copy.

Your request must include your full name, your mailing address, area code and daytime phone number, and your date of birth.

If you do not qualify for the free report, you must remit a fee of $9.50.

Mail your request to:

USIS
PO BOX 33181
Tulsa, OK 74153

You will receive a copy of your report in approximately 5 business days from date received by USIS.
