Truck vibrates at highway speeds... :(

I was going uphill with 43,000 lbs today on Hwy 401 in Ontario and all of a sudden, the truck started shuddering. I cut the cruise control and as the RPM went down, the vibrations disappeared. They reappeared though as soon as I tried to add fuel. I barely made it to the yard which luckily was only 10 miles away. Our shop mechanic at first thought it was the u-joint, but hearing the engine idle, the recommended taking it to CAT. It clearly has a miss. I booked an appointment for 7 am tomorrow and CAT said over the phone it can be either an injector or a fuel supply problem.
Anyone had something like this happen to their truck? Mine is a 2004 International 9400 with a CAT C15 and only 470,000 miles.

Vibrations stay when I'm idling and am in neutral gear. Must be the engine. CAT is looking into now. But it feels like when you have water/ice in the fuel in winter, only now it's summer and there can't be any ice in the fuel system.

No, it was changed 8,000 miles ago but CAT did a cylinder test this morning and found Injector #6 "not responding". $1,000 and 3 hours later the truck was as good as new. It started pulling much better too ... I think this injector has been failing slowly for some time now but this was the first time when I had such a bad misfire/miss. I don't know how I managed to pull the trailer with 43,000 lbs to the yard on 5 cylinders...
