The GPS will show the distance from your location to another, but initially its "as the crow flies". Is it based on this, or actual mileage?

No, if you actually click on "GO" and the gadget does the route, then you see the actual distance. In my case (GARMIN C330) I can only choose between "shortest" and "fastest" mode but the distance is pretty accurate. I don't know how I drove before without it. It's like having your own personal navigator I once sat at a dock in the middle of an industrial area and decided to go grab something to eat. I went into Receiving and asked the guy there if he knew of any fast food places around. The guy said, "Sorry I don't know this area - I live 50 miles away. I only work here...." Then I remembered my GPS (I had just bought it); went into the truck and clicked

WHERE TO
FOOD
FAST FOOD

Within practically 20 seconds the unit found a McDonald's 1.5 miles away! I dropped the trailer and bobtailed to this fast-food joint following the spoken directions of my "navigator".
