As far as I know they will use anything you give them but I have always used the form they provided. They are really laid back and casual so I can't imagine it would be any big deal on way or the other.

Keep in mind now folks, if you're a driver that's been around a bit, or a driver thats new to the road...PROTECT your CDL. Example, I take a medication that does NOT effect my driving in any way. I took a physical recently for an employer. Passed through their orientation, road test, drug test, physical, all of it! NO PROBLEM. I don't drink, I dont do drugs, BUT I am on a med for blood pressure. I was directed to do my Medical Exam at Concentra. They passed me alright, BUT not without taking a peak at my medication history (without my knowledge.) Even though I passed, they still required a medication history, showing current meds, and recently discontinued meds. Until then, my Med Card was only valid a few days. Now, because of that pot of worms opening, I cannot work until I have satisfied all their ridiculous needs. No pay, no job, never and accident, or ticket, never a bad DOT inspection, none of it. But, because of all these newer mandates, the help of some sneaky examiners and all my medical info being available to virtually anyone, the BS Starts! Be careful if you're not revealing meds. Think ahead, keep your card current (using smarts) and don't let these chains of do-gooder's jeopardize your wellbeing! REMEMBER...Concentra WILL check your med history before the examiner even walks in the room to see you! I'm sure they'll get a nice pat on the back for disqualifying drivers! These doctors and clinics are only doing what the feds tell them. I know this is an older thread, but if any of you plan on getting a physical, you need to make sure that your doctor or clinic is on the fmcsa approved list. If not, your medical card will not be valid. We also now need to send a copy to our state DMV. Failure to comply prior to your current card expiring could result in your CDL being downgraded to a class D. I recently got a new physical and uploaded my new cab card directly into the state DMV file. Notices are supposed to be sent out about 90 days prior to your current card expires. Don't let it go. You cannot afford to lose your CDL. I had to set up an account online in order to upload the document. I could have faxed or taken a copy to my DMV. Every time I go to the DMV it takes hours to get to the head of the line. I think all states are set up the same. Remember to keep a copy of your medical card with you. Eventually, we are supposed to not have to keep a copy, but right now we need to have a hard copy with us.

Might want to check your blood pressure first at a drug store self-check machine for free. Mine was sky high. Found out the baking soda I was taking for stomach acid was spiking it (sodium).
I cut out the baking soda, got my BP down then took my physical at Concentra, and got the 2 year version. Paid cash $89.00. I faxed the new medical card into DOT, called to confirm they got it. All done. |
