I need a printable Monthly Log Summary Sheet

The company I now work for uses loose leaf logs and didn't supply me with a monthly log summary sheet. I still have to fill out the 70hr/8 day info on the right side of the log but am accustomed to using the monthly log summary sheet that is on the inside cover of most log books.
I've scoured the internet looking for a copy to print. Nothing. Could anyone please help me? I just need a link to where it's at. It'd make my life a whole lot easier. Thanks in advance! Travis

Try www.jjkeller.com They make most log books. If you can't get it on line stop at a Pilot or other truck stop and buy a log book. You can copy or scan the inside page.

You could always stop by a truckstop and buy buy yourself a log book and then copy the recap. Also, for some reason, I am thinking I have seen recaps by themselves in with the log supplies at some truckstops.
