The doctor will have to be able to give you a DOT approved med card.
Why won't the company you're going to drive for give you the physical?

The price of the physical will depend on what the doctor or clinic charges.
Currently a "regular" doctor, nurse practictioner, physician assistant can give DOT physicals if they are comfortable doing so. Many family doctor's do not have the form on file but you can download and print out the complete package from http://www.fmcsa.dot.gov/forms/forms.htm

I live in NYC and Physical was $115.00, the permit fee at DMV was $40.00 and an additional $10.00 for the endorsements. Good luck to you future driver!!
