I happened upon an unusual sight today at the Love's in Toms Brook, VA, and it made me think of this thread.
There was a flatbed backing up behind a Swift truck. Notice anything wrong here?

Let's zoom in a bit closer, shall we?

You know, after that accident I had, I sort of feel good that I didn't screw up in such, simple ways these guys did.
Seems most of this was due to the lack of basic, common sense.
While mine was the result of ignorance and working too hard in a short-week.
