Default Medical Examination Report

When I completed my medical form I marked chronic low back pain because of an injury in 1979 with surgery in 1986. Chronic meaning that it has been there for a long time. I have no restrictions or limitations.
I am beginning to think that this notation may become problematic in the future.

What do you think?

What is the problem when I complete my next medical form with not even making a note of it?

If there're scars, would be hard for a PA or Doc to ignore. Ditto if it was a workers comp issue. Honesty is the best policy. BOL

I'm going to have to fess up just like you Thor. Only because the last 2 years I've been on L&I and need to complete the gap in my 10 year employment history. Just hope it doesn't bite me in the butt. 5 scars from 5 surgeries in the last 2 years.

Let me know what happens, so I can prepare myself.
Should be starting class in Feb for my CDL
