Fuel and maintance changes for each year and type of truck (also who drives it and how) Insurance is going to very depending on coverage and driveing record.

you cant go by what others costs are. some are better and some or worse then others. my fuel milage for a "crappy" mbn motor isnt bad.. 6.0-6.5mpg others its in the 5's.. some just have bad luck with break downs.. some dont.. only one way to find out.

I don't own a truck, but there's a crapload of items:

Fuel
Truck Insurance
Workers Comp Insurance
Maintenance
Tires
Truck Washes
Taxes
Your personal pay
Your drivers pay (if you're not driving)
Permits

and the list goes on and on. Some items are fixed costs (such as insurance & permits), while others fluctuate, such as tires and maintenance (in the event of a breakdown).

It probably also depends on the carrier you're leased to - I know Celadon requires it.
