Go To: http://www.fmcsa.dot.gov/rules-regul...k-examples.htm Then click Driver's Daily Log This shows the minimum information D.O.T. requires.

As to logging consecutive 24 hour periods.

§395.8 Driver's record of duty status.
(f)(10) Recording days off duty. Two or more consecutive 24 hour periods off duty may be recorded on one duty status record.

Don't make it any harder than you have to unless your company requires more information.
