Accident Register Inquiries

Hello everyone,

I was wondering if anyone could answer a few questions for me about the accident register.

On the Accident Register form that I downloaded from the FMCSA website it says "Index Number" in one of the columns. Is this a number that is given to us by the insurance company or is this a number we make up for organizational purposes? What does it mean?

Second, where should I file/organize the state/insurance reports? Do they go with the accident register?

Thank you in advance for your answers.

Forget their paper form. Electronic file is easier. Do it by the year, month, day, driver. Scan everything pertaining to the accident. Use it for the non-DOT recordable, incidents, ect. In an audit they will ask for a specific accident, everything will be there.

Index # is the one you assign to an accident.
