Oh the driver gets in plenty of trouble if it was something that was truly preventable and/or someone is hurt or killed.

I know an O/O that left his PTO on and his dump body went up going down 95. Hit an overpass and took the dump body right off the truck. It was an alum body so it was softer then steel.
