I've never seen anything that said I had to keep the long form with me at all times while driving. Medical Card suffice's for the long form.

Correct. You don't need to have a copy of your long form medical. You only need your medical card.

Actually, that statement is true only to the point that the Carrier truely did pay for the physical. IF the Carrier deducted the cost's of the physical from a driver's wages (as so many do), then the driver in fact, paid for the physical!

Not true for all states!! Indiana does not do the med. cards anymore, they only have the long forms!!!!

The driver is given a copy of the long form medical form upon completion of the physical for his or her records anyway, so I still don't see any reason why they wouldn't provide one to the driver if asked. The information is private between the doctor, the carrier, and the driver.
That's right.
The driver gets a copy, and they send a copy to the company office. (that's how our clinic does it.)
