The 4 non D.O.T Preventable accidents/incidents are:
A)12/28/04 Dallas TX Ran over landscaping rock.
B)12/07/05 Houston TX The incident happened while I was attempting to back into a dock space. Before, I backed in I got out and looked to make sure the space was clear of debris, and also to see if I was clear on both sides of the truck., I then set my brakes, turned on my emergency flashers and exited the truck . I opened my trailer doors and secured them, and as I was getting back into the cab of my truck, I heard a loud, scraping sound and saw that the driver in the dock space to the right of me had knocked the mirror off of his truck with my trailer door .I immediately reported this to the dispatcher and to the safety department. The other driver then jumped out of his truck demanding money right there on the spot and I explained to him that he would have to file a claim with the company in order to be compensated for his damage. I gave him my driver information but when I asked for his he could no longer speak English. I was later terminated from my position because of this incident.
C)12/07/05 Houston Tx While leaving shipper I hit a parked trailer to avoid running over an elderly man on a backhoe as he drove out from between 2 parked trailers.
D)11/27/06 Maryland Heights MO knocked step off the right side of truck whle backing into a narrow dock area.
The moving violations were both speeding tickets (which have been paid) they occured:
A)06/25/05 Milford TX65 mph in a 55 mph zone.
B)05/19/06 La Grande OR 69 mph in a 55 mph zone. |
