Just about any company you apply to will get a report from USIS dba DAC. I have applied to a few companies and after about 2 weeks I get an envelope in the mail from USIS. It is a copy of the report they generated for XYZ company, and they tell me who XYZ company is.
As for requesting one you may have two options:
1 If you apply to a company ask them for a copy of what DAC sends them...I'm sure they will refuse but remember, it is info about YOU and you do have a right to see it. Or,
2 You can contact USIS (just google them...you can get their contact info from their website). I do not know if they charge for it(they probably do, just like the damn credit bureaus) but the DAC is at leaat partially covered by the Fair Credit Reporting Act so you do have rights there, especially if any "adverse action" is taken as a result of what is in the report.

You'll need to provide your home address and telephone number along with all correspondence. Keep a copy of your request for your own files.
